Purpose

Lifeguards are required to provide reporting in addition to LGS SB 2.4. It is important that timely and accurate reporting is completed. Reporting must be completed on a Lifeguards next available break, during a pause in rotations (at discretion of the Lifeguard in-charge) or at the end of their shift.

The following instances require additional reporting:

  1. Major First Aid
  2. Minor First Aid
  3. Rescues – refer to LGS SB 2.5
  4. Workplace Injury – refer to LGOM 008.
  5. Workplace Health & Safety concerns – refer to LGOM 008.
  6. Complaints – refer to LGS SB 5.14.

There are multiple different systems utilised for the additional reporting:

LIMSOC

Lifesaving Incident Management System and Operations Console (LIMSOC) is designed to streamline the lifesaver and lifeguard sign on/off process and provide more efficient, spatially aware incident management. LIMSOC information feeds back directly to SURFCOM and Lifeguard leadership who can access reports on attendance, rescues, first aid presentations, and more.

JotForm

JotForm is an online service which is used to build and customize forms for various needs, such as surveys, registrations, contact forms, and application forms. The South Bank Lifeguard supervisor has created several JotForm’s for different purposes to allow for data collection outside of the LIMSOC interface.

Procedures

Major First Aid Reporting
Definition: When a patient has an injury or illness which requires medical referral, ambulance attendance, or a where a patient who requires one of the previous leaves against medical advice.

Examples include large wounds which need stitches, post-rescue patients in poor condition, spinal injuries, resuscitation, anaphylaxis, and seizures. Most mental health emergencies will be considered a major first aid (guidance on mental health emergencies is provided in LGS 5.24).

Procedure:

  1. Gather patient (Name, DOB, Address), injury, and treatment details. This should be a thorough medical history i.e. SAMPLE.
  2. Contact SURFCOM (via phone or GWN radio during operational hours – GWN as per LGS SB 5.7) or the South Bank Lifeguard Supervisor (out of SURFCOM hours) to create an Incident Report Log on LIMSOC.
  3. Complete LIMSOC incident report log.
  4. All major first aids should be reported to the Lifeguard Supervisor to ensure staff can be properly debriefed by peer support officers if required

Minor First Aid Reporting

Definition: Where treatment is provided for an injury/ illness which results in no need for medical referral or further observation.

Examples include small cuts, abrasions, or scratches.

Procedure:

  1. Gather patient (Name, DOB, Address), injury, location, and treatment details.
  2. Utilising the SLSQ iPad:
    1. Log into LIMSOC and complete the minor first aid log.
    2. Open JotForm and complete a minor injury report.
  3. Write into the daily report log as per LGS SB 2.4.
