Vehicle, ATV and Jet Ski Checklists
There are 3 sections in the vehicle logbook, one for SSV’s, one for 4WD’s and one for Jet skis. These checklists are to be completed daily to ensure the vehicle/ craft is safe and operational for use. Lifeguards are to ensure that they follow the instructions in the logbook and in each category.
4WD Vehicles, SSV’s and Jet Ski’s will be ‘tagged out’ if they don’t meet certain requirements (This will be expected to occur more regularly). When this occurs, lifeguards are to contact their supervisor immediately to notify them of the issue and log a job in Spiceworks as per normal procedure. Lifeguards should be aware that replacement vehicles may not able to be changed over straight away. Lifeguards may need to set up to their beach without this equipment for a timeframe, whilst a replacement is sourced.
Incident Reporting
Incident report logbooks must be used for mandatory reporting of incidents.
- All major first aids
- Minor first aids (very minor first aids can be recorded in the daily stat on LIMSOC)
- All major rescues
- All search and retrievals
- All workplace injuries to the Lifeguard/s or other SLSQ employees (to be also entered to Folio– using the QR scanner in the daily diary)
- Hazards (Entered into Folio – using the QR scanner in the daily diary)
- All drownings
- All complaints (e.g. from the public about a patrol)
- All resuscitations
To be able to complete an incident report form lifeguards must first notify the State Communication Centre as to the nature of the incident and if an incident report form appears then the lifeguard will be required to complete it. There are a number of incidents that will not require an incident report form to be completed. If a lifeguard thinks that they should have filled out an incident report and none appears they should contact their supervisor.
If a lifeguard is injured the injury must be recorded as an employee injury and its severity. The employee will also be required to complete the SLSQ WHS, Environmental, Security or Property Incident Report Form This form is to report an Injury / Illness, Near Miss, Wellbeing, Safety Concern, Property Equipment, Environmental, or Security incident. If the incident is serious or an employee requires external medical assistance, please immediately notify your Lifeguard Supervisor.

What Is First Aid?
“First Aid” means the provision of first aid facilities, services and personnel required for the initial treatment of persons suffering injury or illness at a workplace.
When Should First Aid Treatment Be Reported?
It is important that any incident that requires First Aid is reported.
Procedure
All first aid reporting must be completed using LIMSOC. The incident report can be completed by first informing SOC of the incident as soon as practical. Lifeguards will then have the following tab appear on the home screen:
Once this screen appears and the lifeguard presses the “create incident” they will appear on the following page which will enable them to complete the incident report form:
