30.2.4. LGS SGC 2.0.4 Daily Report Writing

Purpose

It is the responsibility of the Lifeguard to record all information for their shift. This is to be done in LIMSOC. Please record the following in LIMSOC or in the Daily Diary. All injuries and incidents MUST be recorded via The Star reporting system with Security.

Procedure

1. All Lifeguards
   1. Date/Day
   2. Hours of Work
   3. First Aid (Type, Cause, Medical Attention) – Any and all first aids must be reported with a minimum of age, name, where the injury occurred, location and nature of injury and treatment provided being recorded. Minor First Aids are first aids that require no further treatment and are something that takes lifeguards minimal time to treat. These can be entered in the minor first aid log.
   4. Preventative actions
   5. By-Laws
   6. Public Relations
   7. Rescues Performed (Type, Time)
   8. Missing Persons

2. Senior Lifeguard
   1. Weather Conditions
   2. ‘Water Area’ attendance
   3. Pool closures or extension of hours

If the Lifeguard is not a Senior Lifeguard/Shift Senior they are still responsible for the filling out of the last 3 sections.

3. When beginning a shift, Lifeguards are to fill out their name and starting time only. On completion of the Lifeguards shift they are to fill out the remaining areas.

4. When completing the ‘Daily Diary’ all information is to be written neatly. Accurate information is necessary. It is to be filled out using ink pen only.

5. When completing the ‘Daily Diary’ all sections must be filled out. No section is to be left blank. If it is the case where there is nothing to report about a certain area, Lifeguards are to place a dash or line through this section to indicate this.

Lifeguard/s reporting in LIMSOC.

1. Senior Lifeguard

• Sign On / Sign Off Stats
• Patrol Stats
• Create Incident Report if Supervisor is not on site.
• When Signed off put IPAD/Phone on charge.

2. All Lifeguards

• Any Minor Rescues – Record Rescue
• Any Major Rescues – Record Rescue and Incident Report
• First Aid that Require Incident Report form/Created by Senior Lifeguard

Note: Any Incident Report Forms done on LIMSOC need to be documented into the Daily Diary.
E.g. 1 x IRL in LIMSOC.
