What is an Incident?

‘An incident is a sequence of events that may lead to injury or illness to persons or damage to property or equipment’.
Incidents also cover near-misses which are events which do not lead to injury, but which might reveal a needed improvement in the environment or work practices to avoid future injury.
If you are reporting an incident that requires immediate maintenance or attention you must immediately report it to ‘Surf Com and Rove’, who will notify the relevant parties to ensure the hazard is made safe.

When should a First Aid be reported:

  • All major first aids
  • Minor first aids
  • All major rescues
  • All search and retrievals
  • All workplace injuries to the Lifeguard/s or other SLSQ employees
  • All drownings
  • All complaints (e.g., from the public about a patrol)
  • All resuscitations

If a lifeguard is injured the injury must be recorded as an employee injury and its severity. The employee will also be required to complete the SLSQ Incident Form and forward on to their relevant Lifeguard Supervisor.

First Aid Reporting

What Is First Aid?
“First Aid” means the provision of first aid facilities, services and personnel required for the initial treatment of persons suffering injury or illness at a workplace.

When Should First Aid Treatment Be Reported?
It is important that any incident that requires First Aid is reported.

Procedure

All first aid being a minor or a major incident must be completed using LIMSOC (Either minor first aid or IRL for majors e.g., Any incident that requires further medical assistance.

Minor First Aid

What is a Minor First Aid?

A treatment to an injury where a no further observation or further medical treatment is required. E.g., cuts, abrasions, scratches.

How to report a Minor First Aid

  1. Gather details of First Aid on scene via your notebook in bumbag and record in LIMSOC under minor first aid.

Major First Aid (IRL Required)

What is a Major First Aid
A treatment to an injury where further observation and treatment may be required I.e., open wound, large laceration, medical condition, resuscitation.

How to report a Major First Aid/ Major Rescue requiring First Aid

  1. Contact Surf Com with details to create IRL. Once the IRL is created you will be able to fil or “IRL required’ in LIMSOC filling in the relevant details regarding the incident Record the date, time, weather and specific location of the incident.
  2. Patient’s details including; gender, D.O.B, Age, address, contact details, medical history, any medications and last time they ate.
  3. Details of how the incident occurred and the patient’s activity at the time The patient’s condition or injuries
  4. Any Treatment provided. (IF oxygen or a resuscitation was undergone 2 extra pages will appear)
  5. Who first reported the incident and who attended the incident.
  6. Any referral given. i.e., QAS, medical practitioner, hospital etc
  7. Any other services or personnel who attended, including Security Officer, QPS etc
  8. Treating and completing persons details.

Lifeguards will then have the following tab appear on the home screen

Once this screen appears and the lifeguard presses the “create incident” they will appear on the following page which will enable them to complete the incident report form:
