LGS SB 2.4 Daily Report Writing

Purpose

It is the responsibility of the Lifeguard to record all information regarding their shift. This is to be done in the ‘Daily Report Form’ and LIMSOC ( on the IPad ) (Located in First Aid Room). The ‘Daily Report Form’ consists of the following information;

When completing the ‘Daily Report ’ all required sections for your location must be filled out. No section is to be left blank. If it is the case where there is nothing to report about a certain area, Lifeguards are to place a dash or line through this section to indicate this.

Please note: Refer to LOP 2.5 Incident and First Aid reporting and LOP 2.6 Rescue Reporting to ensure you are following the correct procedure on documents required for reporting.

Procedure

1. All Lifeguards
   1. Date/Day
   2. Hours of Work – Beginning of a shift, Lifeguards are required to fill out their name and hours of work. On completion of shift they are to fill out the remaining areas.
   3. First Aid (Type, Cause, Medical Attention) – Any and all First Aid must be reported with a minimum of age, name, location in the parklands, location and nature of injury and treatment provided being recorded. Minor First Aids are first aids that require no further treatment and are something that takes lifeguards minimal time to treat. These can be entered in the minor first aid form on JotForm. (Refer to LOP 2.5).

   6. Public Relations
   7. Rescues Performed (Type, Time)
   8. Missing Persons

1. Lifeguard 9’s are required to complete
   1. ‘Lagoon Area’ attendance
   2. Extension of hours
      If the Lifeguard is not a Senior Lifeguard / Shift Senior they are still responsible for filling out of the last 3 sections.
2. When completing the ‘Daily Report Log’ all information is to be written neatly. Accurate information is necessary. It is to be filled out using ink pen only.

Lifeguards reporting into LIMSOC.
Procedure

1. Lifeguard 9
   • Sign On / Sign Off Patrol
   • Patrol Stats (How to do Patrol Stats: LGOM 8.2 Daily Patrol Stats reporting)
   • Create Incident Report if Supervisor is not on site.
   • When signed off put IPAD on charge.
2. All Lifeguards
   • Any Minor Rescues – Record Rescue (LIMSOC and JOTFORM)
   • Any Major Rescues – Record Rescue and Incident Report (LIMSOC IRL)
   • Any Minor First Aids- Record Minor First aid (LIMSOC and JOTFORM)
   • First Aid’s that require an Incident Report form (IRL) ae to be created by a Lifeguard 9. If Lifeguard 9 cannot create please contact Lifeguard Supervisor to create IRL>

Note: Any Incident Report Forms done on LIMSOC need to be documented into the Daily Diary.
E.g. 1x IRL in LIMSOC.
E.g., 1 x Rescue in LIMSOC
E.g., 1x Minor FA in LIMSOC
