We refer to the form that recruiters send to hiring managers or clients to receive their opinion about an applicant or candidate as a “Feedback Form.” It looks like this.

To create a new one or edit it, navigate to the advanced settings in your system.

Go to Customizations and then Customize the System.

Locate the feedback entity, which is a native entity from Microsoft out of the box in Dynamics 365. Then, select Forms. You can select a created form or create a new one.

First, press the Save As button and name your feedback form.
Customize the feedback form by adding or removing fields based on your requirements.

Then click the Enable Security Roles button. We recommend marking DCRS Admin and DCRS Recruiter. Press OK to save it.

You can remove or add new fields. We recommend not removing them. If you don’t want certain fields, you can hide them. To do this, group them in the same section and double-click. Then, hide them. Press OK to save the changes.

To create a New Field, click the New Field button.

You will see a pop-up. Put the question or theme that you want to require on the Display Name Field. Then choose if you want a text response, a multi-select option set, a close option set, an image, etc. If necessary, add the options below. Finally, press Save and Close.

To see this new field recently created, click the New Field button and close the popup. Then select it, drag and drop it into the form section.

Press Save and Publish.
