The intent of the LTF tool is to allow centers to indicate a recipient has been lost after all contact attemps have been completed. This includes attempts to obtain records from outside institutions.

Although the tool is intended to reduce burden for centers, it should not be used the first time a visit is considered lost to follow-up. CIBMTR has implemented a prompt in FormsNet3 for the tool to be utilized after two consecutive visits are marked as LTF.

If survival information is known, but remaining clinical data to complete a form is not available, the Survival Tool should be used.
