Query Report

The CIBMTR Query Report is a summary of unresolved queries in FormsNet3 where CIBMTR has identified a potential data issue or is requesting supporting documentation. Transplant centers will receive a CIBMTR Query Report on a weekly basis (Tuesday) and are expected to review the report to determine queries to be resolved. These queries, along with instructions, are placed and need to be resolved in FormsNet3.

*Note: All transplant centers will receive a weekly CIBMTR Query Report regardless of if a query is identified. If the summary counts (Table 1-3) and unresolved query grid (Table 4) indicate “0”, your center can disregard as there are no queries to resolve.
