FormsNet3 does not have a record of the HCT reported on the CTA list. For these cases, a CRID exists, and the infusion will need to be reported in FormsNet3.
Steps to Follow to Correct Discrepancy
- For a patient’s first infusion, complete the following
- Submit an Indication for CIBMTR Data Reporting (2814) form
- Submit the Pre-TED (2400) form
- How to complete the Pre-TED 2400 form
- NOTE: Center will need to provide the patient’s consent for research before the Pre-TED (2400) can be completed
- How to complete the Consent Tool
- How to complete the Pre-TED 2400 form
- For subsequent infusions (for the same patient), complete the following
- Complete the follow-up (2450, 2100, or 4100) form for the prior infusion; report “Did the recipient receive a subsequent infusion?” as ‘Yes’. This will cause a F2814 to come due
- Submit the Indication for CIBMTR Data Reporting (2814)
- After submitting the 2814, the infusion type will be determined. From this, determine whether additional forms must be submitted:
- HCT – Submit the Pre-TED form (2400)
- Allogeneic boost – Submit the Donor Cellular Infusion form (2199)
- Autologous rescue – no additional forms need to be completed
