Recipient LTF Tool
Navigation to the Recipient LTF Tool
The Recipient LTF Tool can be found by selecting the recipient tab, selecting Recipient Tools, and then selecting Recipient LTF Tool. Then, enter the CRID in the search box.
An additional way to navigate to the tool is once a LTF has been reported for a recipient, the Recipient LTF hyperlink will display when under the recipient forms page for that particular recipient.
Recipient LTF Tool Grids
There are three grids within the Recipient LTF Tool.
The Recipient Information Grid
This grid is identical to the ‘Recipient Information’ grid found under the recipient forms. The only difference is that unscheduled forms cannot be generated from the Consent Tool, they can only be created within the recipient forms.
The Recipient LTF Grid
This grid will display the current LTF status for the recipient. Selecting the edit button will allow the recipient to be marked LTF or changed to Active if the recipient’s current status is LTF.
The Recipient LTF History Grid
This grid is an audit trail of any changes that have been made to the recipient’s LTF status. Each time a change is made, a new row will appear in the history grid.

Adding Recipient LTF
The recipient LTF status should be added when all efforts to contact the recipient have been made over multiple visits.
To add recipient LTF, select the edit button within the Recipient LTF grid. When this button is selected, a pop-up window will appear with the ability to select LTF and then provide the reason the recipient is LTF. Once all required questions are answered and submitted, the recipient’s status will appear as LTF within the Recipient LTF grid. This will also set all remaining forms in DUE status to LTF. Additional follow-up forms will not be made DUE when the recipient’s status is LTF.
Updating Recipient LTF
If contact with the recipient is re-established after the LTF status has been reported, select the edit button within the recipient LTF grid. When this is selected, a pop-up window will appear with the ability to select Active and then provide the contact date when contact was re-established with the recipient. After the change is submitted, the recipient’s status will appear as Active in the recipient LTF grid and a row will be added to the Recipient LTF history grid. The contact date reported in the tool is used to determine which follow-up form(s) should be made DUE. Forms will not be made due for the period that the recipient was LTF.
