GENERAL DOCUMENTATION
The following list includes all documentation requirements applicable to any performance levels and paths for this Indicator. Organizations will only submit the documentation requirements relevant to their targeted performance level.
- CC-1 Community Connections Policy: Copy of written policy that meets Indicator requirements and includes the organization’s definition of community within the context of this Indicator as well as explains how it came to this definition.
- CC-2 Assessment Documentation: Documentation of the community assessment, including a narrative of how the assessment was conducted and key findings as well as tools (survey, website, meeting minutes, research reports, etc.) used to assess community history, context, culture, needs, etc.
- CC-3 Community Engagement Plan: Copy of the community engagement plan that meets Indicator requirements.
- CC-4 Partnership Strategy Documentation: A brief narrative describing how the organization is connecting to and creating partnerships with at least one local community group. Additional documentation might include meeting minutes, attendee lists, impact reports, etc.
- CC-5 Community Advocacy Documentation: Documentation showing how advocacy issues are identified and demonstrating participation in advocacy efforts, such as participation records, campaign information, impact reports, etc.
