Documentation Requirements

GENERAL DOCUMENTATION

The following list includes all documentation requirements applicable to any performance levels and paths for this Indicator. Organizations will only submit the documentation requirements relevant to their targeted performance level.

• PH-1 Physical Health and Safety Policy: Copy of written policy that meets Indicator requirements.
• PH-2 Safety Assessment and Actions Documentation: Documentation of the protocols in place to assess physical safety risks as well as the actions taken to enhance physical safety.
• PH-3 Physical Health and Safety Narrative [OPTIONAL]: Brief (one- to two-page) narrative describing the implementation of policy elements within the organization.
• PH-4 Ergonomic Assessment and Fulfillment Protocol: Documentation of the protocols in place to assess ergonomic needs, request adjustments, and distribute equipment (e.g., request forms and equipment distribution data).
• PH-5 CDC Workplace Health Model Documentation: Documentation detailing each of the steps completed and a narrative describing the strategies for improvement (if applicable).
• PH-6 Wellness Facilitators Documentation: Documentation of the provisions offered to facilitate employee wellness.
• PH-7 WELL Building Certification Documentation: Current certification (i.e., active at the time of documentation submission) showing compliance with the Indicator metrics at the targeted level.
• PH-8 Fitwel Certification Documentation: Current certification (i.e., active at the time of documentation submission) showing compliance with the Indicator metrics at the targeted level.
