GENERAL DOCUMENTATION
The following list includes all documentation requirements applicable to any performance levels and paths for this Indicator. Organizations will only submit the documentation requirements relevant to their targeted performance level.
- WB-1 Well-Being Policy: Copy of written policy that meets Indicator requirements.
- WB-2 Paid Time Off Documentation: Documentation of combined paid vacation and sick time for full-time employees, such as a policy document or benefits handbook.
- WB-3 Bereavement Leave Documentation: Documentation that the organization offers bereavement leave that is separate from paid vacation or sick time.
- WB-4 Well-Being Program Documentation: Documentation (such as a narrative or program collateral) of the comprehensive employee well-being program showing compliance with Indicator metrics at the targeted level, including the planned components, feedback mechanisms, which aspects have been initiated, and strategies for improvement, as applicable.
- WB-5 Well-Being Survey Documentation: Documentation showing compliance with Indicator metrics at the targeted level (i.e., calculations), as well as screenshots or file submissions of survey questions, corresponding results, and survey response rate.
- WB-6 Best Places to Work Award: A copy of a qualifying Best Places to Work Award.
