Documentation Requirements

GENERAL DOCUMENTATION

The following list includes all documentation requirements applicable to any performance levels and paths for this Indicator. Organizations will only submit the documentation requirements relevant to their targeted performance level.

• HC-1 Health Care Policy: Copy of written policy that meets Indicator requirements.
• HC-2 Health Care Coverage Documentation: Documentation of the percentage of health insurance plan premiums covered for employees and that coverage is comprehensive and meets or exceeds government rules and regulations regarding the provision of health care insurance to employees and their dependents, as applicable.
• HC-3 Family Health Care Coverage Documentation: Documentation of both the percentage of health insurance plan premiums covered for employees’ spouses, domestic partners, and dependents.
• HC-4 Universal Health Care Coverage Documentation: Documentation of the universal health care coverage provided to employees.
