Documentation Requirements

GENERAL DOCUMENTATION

The following list includes all documentation requirements applicable to any performance levels and paths for this Indicator. Organizations will only submit the documentation requirements relevant to their targeted performance level.

• FML-1 Family and Medical Leave Policy: Copy of written policy that meets Indicator requirements.
• FML-2 Leave Benefit Documentation: Documentation showing the terms of eligibility and duration of leave allowed, including the percentage of salary paid while the employee is on leave.
• FML-3 Flexible Work Documentation: Documentation of the flexible work arrangement options.
