    • For each disaster type, users can indicate whether it has an impact and, if so, select the appropriate level of impact (e.g., High, Medium, Low) in the corresponding cell.
  2. Comment: A field for adding comments related to the disaster impact.
  3. Submit Form: A button to submit the completed form.
