Question 53: Total number of lines of systemic therapy received

Systemic therapy (chemotherapy) is delivered via the blood stream and distributed throughout the body. Therapy may be injected into a vein or given orally.
Report the total number of lines of systemic therapy (chemotherapy) the recipient ever received in their lifetime, including all systemic therapy (chemotherapy) given for prior malignancies, autoimmune disorders, etc.

Question 54-62: Exposure to anthracycline agents

Anthracycline agents (e.g. Daunorubicin, Doxorubicin, Epirubicin) are a type of antibiotic that comes from certain types of Streptomyces bacteria. Anthracyclines are used to treat many types of cancer. Anthracyclines damage the DNA in cancer cells, causing them to die.

Indicate if the recipient had exposure to any of the anthracycline agents provided in the list in their lifetime. If the anthracycline agent is not listed, select Other anthracycline and specify. If no anthracycline agents ever were given, select None.

Report the total cumulative dose given in the recipient’s lifetime for each applicable anthracycline agent.

Question 63-69: Exposure to alkylating agents

Alkylating agents (e.g. Cisplatin, Cyclophosphamide, Melphalan) keep the cell from reproducing (making copies of itself) by damaging its DNA. These drugs work in all phases of the cell cycle and are used to treat many different cancers, including sarcoma.

Indicate if the recipient had exposure to any of the alkylating agents provided in the list in their lifetime. If the alkylating agent is not listed, it is not necessary to specify. If no alkylating agents ever were given, select None.

Report the total cumulative dose given in the recipient’s lifetime for each applicable alkylating agent.

Question 70-71: Exposure to targeted therapies

Targeted therapy works by stopping or slowing the growth or spread of cancer. Targeted therapies are designed to interfere with, or target, these molecules or the cancer-causing genes that create them. Indicate if a targeted agent was received.

Indicate if the recipient had exposure to any of the targeted therapies provided in the list in their lifetime. If the targeted therapy is not listed, select Other targeted therapy and specify. If no anthracycline agents ever were given, select None.

Question 72: Radiation Therapy

Radiation therapy uses high-energy, ionizing radiation to kill malignant cells. Much like non-targeted systemic therapy, radiation therapy does not specifically target malignant cells and does have significant side effects. For that reason, high-dose radiation often targets a limited field.

Indicate Yes or No if the recipient received radiation treatment in their lifetime. This could be local / regional or non-primary radiation.

Questions 73-74: Local / Regional

External beam radiation therapy comes from a machine that aims radiation at your cancer. It is a local treatment, which means it treats a specific part of your body.

Regional therapy includes a combination of surgery, chemotherapy, hyperthermia (heat) and radiation to target advanced cancers. It is highly effective for tumors that are limited to a specific area (region) in the body.

Indicate Yes or No if the radiation that was given to the recipient was local/regional radiation treatment in their lifetime and specify the total cumulative lifetime dose.

Questions 75-77: Non-primary and site

Non-primary radiation is radiation that originates from a source other than the primary radiation beam, radiation that is not directly produced by the primary radiation source itself. An example would be proton therapy and radiation received by normal or surrounding tissues.

Indicate Yes or No if the recipient was exposed to non-primary radiation treatment in their lifetime and specify the site. If the location of the non-primary radiation is not available, select Other and specify.
