Question 2: Did the recipient have splenomegaly at the last evaluation prior to initiation of hypertransfusion?

Indicate if the recipient had splenomegaly (i.e., abnormal enlargement of the spleen) at the last evaluation prior to the hypertransfusion. Splenomegaly is often documented during the physician’s physical assessment of the recipient and represents an abnormal finding. Splenomegaly can also be detected by imaging techniques such as ultrasonography, CT or MRI.

Indicate Yes if splenomegaly was present at the last evaluation prior to initiation of hypertransfusion.

Indicate No if splenomegaly was not present at the last evaluation prior to initiation of hypertransfusion.

Indicate Unknown if it is not possible to determine the presence or absence of splenomegaly at the last evaluation prior to initiation of hypertransfusion.

Question 3: Specify the spleen size

Indicate the size of the spleen in centimeters, measured below the left coastal margin as assessed by physical exam. If there are multiple spleen measurements, report the most recent measurement prior to the initiation of hypertransfusion.

If the spleen size is not documented prior to the initiation of hypertransfusion, leave the data field blank and override the FormsNet3SM error.

Question 4: Specify the spleen size

Indicate the size of the spleen in centimeters, as assessed by imaging (ultrasound, CT / MRI). If there are multiple measurements, report the most recent value prior to the initiation of hypertransfusion.

If imaging to assess the spleen size not completed prior to the initiation of hypertransfusion, leave the data field blank and override the FormsNet3SM error.

Question 5: Specify the spleen volume

This question is optional.

Spleen volume should be found on CT / MRI report or can be calculated by the radiologist. If the spleen volume is not documented, seek physician clarification. Data managers should not calculate the spleen volume.

If there are multiple scans, report the spleen volume using the most recent scan prior to the initiation of hypertransfusion.

The spleen volume can be calculated using the spleen measurements obtained from a CT / MRI and the standard prolate ellipsoid formula.

Specify the spleen volume in the current reporting period, as documented by the physician. If the spleen volume is not documented, seek physician clarification. Data managers should not calculate the spleen volume. If there are multiple scans, report the spleen volume using the most recent scan prior to the initiation of hypertransfusion.

If a scan was not completed in the reporting period or the physician determines the spleen volume cannot be calculated, leave the data field blank.
