Question 74: Was there recurrence of the primary non-malignant disease post-infusion?

Recurrence of non-malignant disease is defined as return of symptoms related to the primary non-malignant disease for infusion or recurrence of the primary non-malignant disease for infusion as determined by the treating physician.

Indicate if the recipient had recurrence of the non-malignant disease during the reporting period. If it is unclear if the non-malignant disease recurred, seek physician clarification.

Question 75 – 76: Was the date of recurrence previously reported?

This question is intended to capture the first instance of disease recurrence. Specify if the date of recurrence was reported in a previous reporting period.

If the first instance of recurrence is within the current reporting period, select No and report the assessment date that first showed recurrent disease. If the date of non-malignant disease recurrence is unclear, seek physician clarification.

If the exact date is not known, refer to General Instructions, General Guidelines for Completing Forms, for information about reporting partial or unknown dates.
