9.8.2.3.1. Investigation

In this tab, users can conduct an investigation of the root cause.

!In order to access the contents of this tab, users will need to have selected the Validated Supplier Responsibility checkbox in the Create SCAR form.

1. Problem Statement: This is a rich text field that allows users to input the problem statement.
2. Detailed Investigation: This is a checkbox which, if enabled, allows users to add a detailed investigation.
   • If selected, the content inside the Analysis tab will be made visible.
3. Root Cause Description: This is a rich text field that allows users to input a description of the root cause of the problem.
4. Root Cause Documents: This is an attachment field that allows users to attach any documents related to the root cause.

5. Unable to Determine Root Cause: This is a checkbox which, if enabled, allows users to indicate that they are unable to determine the root cause.
6. Submit Form: This button allows users to submit the form.
   • Once submitted, the SCAR will be sent for review.
