9.8.2.3.2. Analysis

In this tab, users can perform an analysis of the root cause. Specifically, users can apply the 5Ws to 3 types of causes/failures—a method known as the 3×5 Why Analysis.

*5W (or 5W1H) = What, Where, When, Who, and Why. 3 Causes/Failures = Direct, Detection, and Systemic.

!In order to access the contents of this tab, users will need to select the Detailed Investigation checkbox in the Investigation tab.

1. Direct Cause

• What/Where/When/Who/Why: These are all text fields that allow users to input their answers. They can be expanded to encompass multiple lines.

2. Detection Cause

• What/Where/When/Who/Why: These are all text fields that allow users to input their answers. They can be expanded to encompass multiple lines.

3. Systemic Cause

• What/Where/When/Who/Why: These are all text fields that allow users to input their answers. They can be expanded to encompass multiple lines.
• Submit Form: This button allows users to submit the form.
  • Once submitted, the SCAR will be sent for review.
