A key part of any health and safety management system is the collection of data. It is a requirement under most state workplace health and safety legislations that an employer keep records of all injuries, work caused illnesses and dangerous events. It is also a requirement to inform the relevant government department(s) of any serious bodily injury, work caused illness and dangerous event that occurs in the workplace. The collection of data is to continually improve SLSQ’s processes and procedures.

Reporting
It is the members responsibility to report any injury/incident to the appropriate personnel. It is the Master of the ships (appropriate personnel) responsibility to and correctly record an injury or incident as per incident/injury flowchart. Every injury work-caused illness and dangerous incident must be reported and recorded regardless of the amount of injury or damage (potentially dangerous occurrences should be recorded even if there is no injury or damage).

In addition to actual incidents, identified hazards should also be reported to ensure that corrective action can be taken and to identify potential problems with equipment or procedures.

The Department of Transport requires a Marine Incident Report Form to be completed in the event of a major “marine incident”. A Marine Incident Report Form must be completed and returned to the nearest regional office within 48 hours of the incident.

Duty of Care
All IRB/powercraft personnel have a duty of care to minimise the risk of an incident or injury to themselves, their crew and the bathing public.
Any compromise to this duty of care will initiate a very comprehensive investigation process which may have severe outcomes should the code of conduct and duty of care policies be compromised.

Investigations
If IRB/ powercraft personnel who are found to be in breach of the Safety Management System, codes of conduct and relevant policies will result in an internal investigation.
All SLSQ investigations are internal documentation and are private and confidential.

SLSQ Investigation Officers
The following personnel have been delegated by the CEO to act as the SLSQ Investigation Officer for the following or investigators will be appointed by the CEO as required:

  • IRB Competition and Training – Surf Sport Manager
  • IRB Training – Regional Lifesaving Coordinator/Supervisor
  • IRB Patrol – Regional Lifesaving Coordinator/Supervisor
  • RWC Operations – Regional Lifesaving Coordinator/Supervisor

SLSQ may seek an external investigation officer to investigate the injury or incident.
