Surf Life Saving Queensland would like to recognise a member each month who is an active patrolling member, has shown to other members of the organisation or the wide community exemplary leadership within lifesaving, comradery or represents SLSQ’s values positively. This award will begin in April of 2024 and will be for every month of the year. It is NOT to be confused or related to a rescue or incident; we encourage you to nominate said person for a Lifesaving Excellence Award, if so.
The following questions will be asked inside the JotForm, so prior to commencing the online nomination, please read the following:
1. The nominator name; and
2. An email address you would like to be contacted on as the nominator; and
3. The nominee’s name; and
4. The nominees club; and
5. In your own words, tell us why the person you nominated should be ‘Lifesaver of the Month’ (in 250 words or less); and
6. Upload a photo of the nominee (for social media purposes).
