All Incident reports are to be entered directly into LIMSOC via the Ipad or through Surfcom.

For all major incidents, clubs/services must radio through to the SurfCom to create an incident in the system, additionally an “First Aid Log” (FAL) may be allocated to the incident to allow patient or other information to be added.

FALs must be completed by the end of your patrol shift unless extenuating circumstances apply. If extenuating circumstances apply, the First Aid Log should be completed within 48 hours and Surfcom must be notified of the reasoning and estimated time of completion.

FALs must be completed by the Patrol Member who is administering first aid to the patient.

NOTE: One First Aid Log per Person per Incident

First Aid reports will be completed in LIMSOC and is the mandatory reporting of incidents, which include:

  • All major first aids;
  • All major rescues;
  • All search and retrievals;
  • All member injuries (e.g. around club, at carnivals, on patrol);
  • All drownings;
  • All near drownings;
  • All complaints (e.g. from the public about a patrol);
  • All resuscitations.

NOTE: Minor first aids may also be reported at the discretion of the Patrol Captain.

In the event of a member injury, members must notify Surfcom who will create a First Aid Log.

The injury must be recorded as a member injury and noted if it was a major or minor injury.
