All Patrol Captains, Club Captains and Vice Club Captains shall sign this page where indicated to confirm the receipt and understanding of the information provided in this manual.

Clubs are to maintain the original copy of this club signature page (for future reference) and forward a copy of the completed club signature page to the relevant branch or SLSQ Lifesaving Services Coordinator to confirm that patrol leaders are fully aware of the contents of this manual.
