DISCHARGE CATEGORIES – GUIDE
Examples:
• See Pt. Handbook reasons for Immediate Termination
• Falsified Urine Sample
• Threatening and/or violent/abusive behavior
• Possession of weapons or use of objects as a weapon
• Loitering despite warnings
• Behaving suspiciously in or around clinic grounds
Completed Treatment Medicated through detox program or progressively decreased maintenance
medication to meet a planned completion date.
Deceased Patient is deceased.
Financial / Nonpayment of Fees
Patient unable to meet financial obligations and did not complete treatment.
Hospitalized Patient admitted to a medical or psychiatric hospital.
Incarcerated Patient detained at correctional institution.
Internal Transfer Patient transferred from one to another clinic (Sister Clinic).
Modality Change Patient requires a different level of care, transferring outside OTP Clinic. Examples:
• Pain management
• Inpatient, Residential and/or Abstinence program
Transfer Out Competitor Patient transferred to a site.
Transfer Out Relocation Patient transferred to a clinic beyond 25 miles from any clinic.
Unknown Every effort to identify the underlying reason for discharge has been made.
• CA – System automatically discharges after 14th consecutive missed dose (3 days for
detox)
• TX – System automatically discharges after 14th consecutive missed dose
• GA – System automatically discharges after 30th consecutive missed dose
• GHP – discharged after 3 consecutive missed doses and did not return by the end of
the monthly reporting period
Unsuccessful Treatment Patient did not successfully complete treatment.
Examples:
• Pt. terminated leaving against medical advice (AMA)
• Failed to progress in treatment as outlined in treatment plan
• Failed to attend scheduled counseling sessions
• Refused to enter recommended levels of intensive treatment
• Medical Non-Compliance
