The ASI – Supplemental screen ask the counselor to summarize the assessment interview. The counselor enters a summary on various topics in the Comment boxes. The counselor will summarize and assess in the following areas:
• What gender to you identify with?
• What is your literacy level (reading ability)?
• How would you rate the effectiveness of your current medications? Previous medications?
• Do you use any complementary health approaches such as: natural products and dietary supplements, mind and body practices (yoga, massage, acupuncture, etc.), homeopathy, naturopathy and/or traditional healers? If so, which ones?
• Do you need any assistive technologies in order to access services here, such as vision/hearing impairment, wheelchair, walker, etc?
• What is your military history?
• Have you ever tried to harm yourself (such as cutting, etc..), or took risks that could cause severe injuries or be life-threatening?
