In the Create SCAR form, users can create and customize the SCAR to align with their specific needs and preferences.

  1. SCAR ID: This field is automatically populated with a unique identifier for the SCAR. It ensures that each request can be distinctly referenced and tracked within the system.
  2. Initiation Date: This is a date picker that allows users to input/select the date when the SCAR is initiated.
  3. Response Due Date: This is a date picker that allows users to input/select the date by which they expect a response from the supplier regarding the SCAR.
  4. Supplier: This is a single-select dropdown field that retrieves the list of existing suppliers in the system. It is a mandatory field. It allows users to select the supplier to which the SCAR will be issued.
    • Once a supplier is selected, the following fields are automatically populated with the corresponding information:
      • Supplier ID: This field displays the ID associated with the selected supplier.
      • Supplier Representative Name: This field displays the name of the representative associated with the supplier.
      • Supplier’s Email: This field displays the email address of the supplier’s representative.
      • Supplier’s Phone: This field displays the phone number for the supplier’s representative.
  5. Issue Description: This is a rich text field that allows users to provide a description of the issue or non-conformance that triggered the SCAR.
  6. Accordions: The form includes several collapsible sections, known as accordions, which each serve a specific purpose.
  7. Submit Form: This button allows users to submit the completed form.
