The forms act as evidence that the supervised member has attended the required number of sessions and successfully demonstrated the requirements.
Only a supervisor qualified and endorsed to deliver the award/qualification being supervised can complete the sign-off of an Evidence Form (F33, F34, or F35).
This includes:
- Being a current member of an appropriate age for the course being run
- Remaining current in the base award/qualification
- Remaining current in the base award pre-requisites
- Holding an appropriate Training/Assessment qualification
- Completing (own) necessary Evidence Form/s (F33, F34, F35)
- Completing (own) Trainer, Assessor, Facilitator D9 Matrix
It is noted that additional processes are required for Trainee Trainers, who only hold a Training Officer Certificate (TOC). These provisions are set out in ‘The Supervision Model’ section and are in addition to the Evidence Form process.
