9.1.1.18. Tissue Information

In this section of the Create New Incident form, users can provide information about the tissue involved in the incident.

*This type of incident involves medical events directly related to tissue within the human body, including tissue donation and transplantation.

*For guidance on common form functions, please refer to the dedicated section in the manual.

1. Tissue Name: A single-select dropdown field for specifying the tissue involved in the incident. This dropdown displays a fixed list of tissues, such as Cartilage, Bone, and Skin.
2. Is it a living donor?: A radio button for specifying whether the tissue donation was from a living donor.
3. Age of the Donor: A numeric field for entering the age of the tissue donor.
4. What is the gender of the donor?: A radio button for selecting the gender of the donor.
   • If Self-Description is selected, the following field becomes visible:
     
     • Gender Self-Description: A text field for providing a self-description of the donor’s gender.
5. Relevant Documents (Optional): A file attachment field for uploading documents related to the medical incident.
