9.1.1.14. Environmental or Safety Incident Details

In this section of the Create New Incident form, users can document the environmental or safety aspects of the incident.

*This type of incident involves events that pose a serious risk to the environment, workplace safety, or public safety due to quality-related issues.

1. Did It Result from a Natural Occurrence?: A checkbox for indicating if the incident was caused by a natural event.
   • Selecting this checkbox reveals the following set of options:
     
     • Disease: Select if the incident is related to a disease.
     • Earthquake: Select if the incident is related to an earthquake.
     • Fire: Select if the incident is related to a fire.
     • Flood: Select if the incident is related to a flood.
     • Tornado: Select if the incident is related to a tornado.
     • Other Disaster: Select if the incident is related to another type of natural disaster.
       • Selecting this checkbox reveals the following field:
         
         • Specify the Natural Disaster: A text field for specifying the type of natural disaster.
2. Did It Result from a Non-Natural Occurrence?: A checkbox for indicating if the incident was caused by a non-natural event.
   • Selecting this checkbox reveals the following set of options:
     
     • Chemical Spills: Select if the incident involves chemical spills.
     • Industrial-Based Fires: Select if the incident involves industrial-based fires.
     • Improperly Contained Radioactive Waste: Select if the incident involves improperly contained radioactive waste.
     • Other Disaster: Select if the incident involves another type of non-natural disaster.
       • Selecting this checkbox reveals the following field:
         
         • Specify the Non-Natural Disaster: A text field for specifying the type of non-natural disaster.
3. Describe the Actual or Potential Environmental Impact Resulting from the Incident: A rich text field for describing the environmental impact of the incident, whether actual or potential.
4. Relevant Documents (Optional): A file attachment field for uploading documents related to the environmental or safety incident.
