Once the SCAR instance has been initiated, the assigned user will receive a notification in their Pending Tasks informing them that they have to create SCAR.

Clicking on the subject in Pending Tasks should direct the user to the Create SCAR form. Depending on the values inputted in the previous form, there can be two different versions of the Create SCAR form.

Version 1

If the Detailed Investigation checkbox was not selected in the previous form, the Create SCAR form will appear as follows:

  • Initiation Date: This field can be modified.
  • Response Due Date: This field can be modified.
  • Supplier: This field can be modified.
  • Supplier ID: This field is automatically populated with the Supplier ID specified in the Supplier form. Users cannot modify this field.
  • Supplier Representative Name: This field can be modified.
  • Supplier’s Email: This field can be modified.
  • Supplier’s Phone: This field can be modified.
  • Issue Description: This field can be modified.
  • Submit: This button allows users to submit the form.

Version 2

If the Detailed Investigation checkbox was selected in the previous form, the Create SCAR form will be composed of 2 main sections: Details and 3×5 Why Analysis.

It will appear as follows:

  1. Details
    • Initiation Date: This field can be modified.
    • Response Due Date: This field can be modified.
    • Supplier: This field can be modified.
    • Supplier ID: This field is automatically populated with the Supplier ID specified in the Supplier form. Users cannot modify this field.
    • Supplier Representative Name: This field can be modified.
    • Supplier’s Email: This field can be modified.
    • Supplier’s Phone: This field can be modified.
    • Issue Description: This field can be modified.
    • Submit: This button allows users to submit the form.
