For patients:
Add action plans
- Click on ⊕ in the upper-right corner.
- Add a title to your action plan. Each action plan contains sub-sections called items.
- Click on +Add item. These items serve as sub-sections to your action plan.
> To remove an item in an action plan, click on ‘-Remove Item’. - Fill in the title of your action plan and fill in the input form.
- Click ‘Save’.
Edit action plans
- Locate the name of the action plan you would like to edit.
- Click on
- Click on ‘Edit’.
- Fill in the input form.
- Click ‘Save.’
Delete action plans
- Locate the name of the action plan you would like to delete.
- Click on
- Click on ‘Delete’;
For clinicians:
Add action plans

- Click on +Add item to add a sub-section to your action plan. To remove a sub-section, click on ‘-Remove Item’.
- Click ‘Save’.

- Locate the name of the action plan.

- Locate the name of the action plan you would like to delete.
