For patients:

Add action plans

  1. Click on ⊕ in the upper-right corner.
  2. Add a title to your action plan. Each action plan contains sub-sections called items.
  3. Click on +Add item. These items serve as sub-sections to your action plan.
    > To remove an item in an action plan, click on ‘-Remove Item’.
  4. Fill in the title of your action plan and fill in the input form.
  5. Click ‘Save’.

Edit action plans

  1. Locate the name of the action plan you would like to edit.
  2. Click on
  3. Click on ‘Edit’.
  4. Fill in the input form.
  5. Click ‘Save.’

Delete action plans

  1. Locate the name of the action plan you would like to delete.
  2. Click on
  3. Click on ‘Delete’;

For clinicians:

Add action plans

  1. Click on ⊕ in the upper-right corner.
  2. Add a title to your action plan.
  3. Click on +Add item to add a sub-section to your action plan. To remove a sub-section, click on ‘-Remove Item’.
  4. Click ‘Save’.

Edit action plans

  1. Locate the name of the action plan.
  2. Click on

  5. Click ‘Save’.

Delete action plans

  1. Locate the name of the action plan you would like to delete.
  2. Click on
