Option 3: 3×5 Why Analysis
Users can select this checkbox to apply the 5Ws to 3 types of causes/failures.
If selected, it will add the three following fields to this section:
- Direct Cause
- What was the DIRECT or specific cause of the problem? Explain why the problem occurred (e.g., design/drawing error, manufacturing process, assembly/installation instructions).
- What/Where/When/Who/Why: These are all text fields that allow users to input their answers. They can be expanded to encompass multiple lines.

- What/Where/When/Who/Why: These are all text fields that allow users to input their answers. They can be expanded to encompass multiple lines.
