8.2.2. Provide Analysis Documentation (Optional)

In this section of the form, users can document their analysis findings and leverage various tools to support the risk assessment.

1. Summary of the Analysis: This is a rich text field that allows users to summarize their analysis findings, including key insights, conclusions, and recommendations derived from the risk assessment.
2. Provide Analysis Documentation: This is a file attachment field that allows users to upload documents, reports, or other materials pertaining the analysis.
3. Analysis Tools: This section offers tools to aid in identifying and analyzing risks.
   1. Draw a Diagram: This checkbox allows users to create a visual representation of the risk analysis.
      • If selected, the free draw control, a digital tool for creating freehand drawings, will become visible.
        

        **To draw, simply drag your cursor or stylus across the drawing area. To erase or undo your drawing, click the button.

   2. Define the SWOT: This checkbox allows users to conduct a SWOT analysis.
      • If selected, rich text fields for Strengths, Weaknesses, Opportunities, and Threats will become visible, enabling users to document each component.
        
   3. Use the FMEA Method: This checkbox allows users to perform a Failure Modes and Effects Analysis (FMEA).
      • If selected, the following grid will become visible:
        
        • Add: This button allows users to add a new row to the grid, revealing the following input fields:
          
          • Process Step: This is a text field that allows users to specify or describe the process step being analyzed.
          • Failures: This is a text field that allows users to identify potential failure modes or ways in which the process step could fail.
          • Effect: This is a text field that allows users to describe the potential effects or consequences of each failure mode.
          • Cause: This is a text field that allows users to identify the root causes or reasons behind each failure mode.
          • Severity: This is a text field that allows users to rate the severity of each failure mode.
          • Occurrence: This is a text field that allows users to rate the likelihood of each failure mode occurring.
          • Detection: This is a text field that allows users to rate the likelihood of detecting each failure mode before it causes an impact.
            

            **The Severity, Occurrence, and Detection ratings are typically assessed on a scale of 1 to 10.

        • Discard Changes: This button allows users to discard their changes.
        • Save Changes: This button allows users to save their changes.

          Upon saving the row, the Risk Priority and Criticality values are automatically calculated based on the Severity, Occurrence, and Detection ratings assigned to each failure mode.
