The following is some guidance for filed formatting in Claim Maker:
ALL CAPS
ALL text fields should be entered in ALL CAPS unless otherwise specified.
Addresses
Addresses should be entered as they appear in the source documentation in ALL CAPS.
Phone Numbers
All phone numbers should take the form “+1 ###-###-####”.
Names
Names of all types should be recorded in ALL CAPS. If the field in question is NOT split into individual fields for each name element (e.g. first, last, middle, etc.), enter the name in “LAST, FIRST [MIDDLE], [SUFFIX, …]” format (e.g. Jane Q. Doctor, MD, DO “DOCTOR, JANE Q, MD, DO”)
Dates
All dates should be entered in YYYY-MM-DD format (e.g. January 2nd, 2022 “2022-01-02”)
Times
All times should be entered in hh:mm format using 24-hour notation for the hour (e.g. 3:15 PM “15:15”)
