The Written Agreement Between Student and Agency

Once you have been offered a practicum please go to the D2L site. Locate the Practicum confirmation Google Doc. Fill in all of the required information including the name of your practicum supervisor(s) who will sent a copy confirming your placement.

The Practicum Coordinator will then send out a welcome package to the Agency.
