9.5.4.1.1. Risk Scoring System Guide

In both the Quality Event application and EPC, risks are scored according to the following system:

Impact

• Very Low: Score = 1
• Low: Score = 2
• Medium: Score = 3
• High: Score = 4
• Very High: Score = 5
  

Probability

• Very Low: Score = 1
• Low: Score = 2
• Medium: Score = 3
• High: Score = 4
• Very High: Score = 5
  

Detectability

• Very Easily Detectable: Score = 1.
• If Easily Detectable: Score = 2
• If Moderately Detectable: Score = 3
• If Hard to Detect: Score = 4
• If Very Hard to Detect: Score = 5
  

Total Score = (Impact Level Score) × (Probability Level Score) × (Detectability Level Score)
