12.3.3. Root Cause

In this section of the Submit to Supplier form, users can examine and elaborate on the root cause investigation. Users are also granted access to the Review sub-accordion, which enables them to assess the comprehensive impact of the identified root cause.

*For more details on the first three accordions in this section, click here. These accordions, previously accessible in the workflow, will retain any previously entered information, allowing users to modify or expand upon it in this phase.

In the Review accordion, users are provided with a set of radio buttons that facilitate the selection of a single option from a set of mutually exclusive choices. Users can opt for either Yes or No to address the following:

1. Has the root cause identified a fundamental breakdown or failure of a process?: This radio button allows users to evaluate whether the identified root cause signifies a fundamental breakdown in a process.
2. Has the element of the process that caused the root cause been corrected?: This radio button allows users to confirm the correction of the specific process element responsible for the root cause.
3. Are the identified root cause and related corrective action applicable to other parts or processes?: This radio button allows users to determine the broader applicability of the identified root cause and corrective action to other parts or processes.
   • If Yes is selected for this question, it will reveal the following field:
     • If Yes, List Other Part Numbers: This is a text field that allows users to list the numbers of other applicable parts.
