Scenario 1

The supervisor contacts the regional chair in rare unusual circumstances before the 45-day deadline, before supervisor’s evaluation is late. The supervisor negotiates with the student and receives approval from the regional accreditation chair to extend the 45-day deadline. The supervisor’s evaluation will document this process, and such extensions must be reported on the next annual report. No notation is assigned.

Scenario 2

The supervisor contacts the regional chair after the 45-day deadline to inform that evaluation(s) are not written or late. The center will be assigned a notation.

a. The regional chair receives the self-report with a complete plan of action to come into compliance and submits it to the Commission so a National Reviewer is assigned.
b. In the absence of a complete plan of compliance, the regional chair adds the center in the non-consent agenda.
