ACPE ACCREDITATION COMMISSION
SITE VISIT REPORT—PART II: FINAL RECOMMENDATIONS OF THE SITE TEAM
This report is due to the center 58 days after the conclusion of the site visit (14 days after the 30 day period for the Center’s Response ends.)

1. Comment on the Center’s written response to the site team visit and Site Visit Report, Part I. Assess the center’s compliance or feasibility for compliance (pre-accredited) with ACPE Standards. Include 2016 Accreditation Manual, Appendix 5 Part I and Part II, providing distinct evidence that the information provided by the center is correct, and make additional comments if necessary to document points of non-compliance.
2. If appropriate, identify areas of non-compliance, citing the specific standard(s) involved.
3. Summarize specific items of concern or issues for review by the Commission reviewer or discussion by the Accreditation Commission.
4. If appropriate, prepare a separate document, listing any commendations (See Appendix P-3, Commendations for Centers) that may be appropriate for the center. Describe the portion of the center’s documentation, operations, or program design and execution found to be exemplary. Specific Standards must be cited for commendations. NOTE: Any recommendations for commendation(s) are shared only with the Commission presenter and the Accreditation Commission Chair. They are not to be made available to the Center.
5. Record the site visit team’s vote on its recommendations to the Commission.

Instructions:
• Please send a copy of the Site Visit Report, Appendix 5, Parts I & II, to the Center and the Area Chair of Accreditation.
• Combine Site Visit Report Part I, the Center Response, Site Visit Report Part II, Appendix 5, Parts I & II, and all Center materials and correspondence, and any recommendations for commendation(s) and submit to the Commission Reviewer assigned by the Accreditation Commission Chair.
