The written agreement between student and agency

Once you have been offered, the Placement Confirmation Form MUST be completed verifying that a practicum has been offered.

This document requires the signature of both the supervisor and you.

After you have both signed the agreement you must submit it to the Practicum Field Director for their signature. NOTE: Your practicum will be confirmed AFTER the Field Director has reviewed and signed the agreement.

You and the agency will receive a copy of the form once this process is complete.

TIP: It is a good idea to have a copy of the agreement printed and to take it with you to the agency interview in the event that the supervisor is able to offer placement on the day of the interview.
