In the event that the center has concerns about a change made during the audit that was not addressed prior to the final audit report, a formal data review process will be initiated. Effective January 1, 2026, data reviews may only be requested for centers that fail their HCT audit at a critical field error rate between 3.1% and 3.3%. In this scenario, the center submits their questions and concerns along with any applicable source documentation regarding the data changes. This information is then reviewed by the audit team. If the changes reviewed result in data field changes and a change to the originally calculated error rates, a summary letter describing the data changes and new error rates are sent to the center along with a detailed explanation about why the change was made. In the case where a center’s pass/fail status is changed based on the data review, an abbreviated amended audit report is also produced and sent to the center.
Centers that either pass their audit or have a critical field error rate greater than 3.4% may submit questions, concerns, and applicable source documentation for auditor review. If the auditor review determines that data updates are warranted, the database will be updated accordingly. However, such updates will not result in adjustments to the calculated error rates, and an amended audit report shall not be issued.
