6. DEPENDENTS

Employees are encouraged to add dependents to the EP. If insurance coverage is requested for family members, dependents are required.

On the homepage select Request Changes.

Choose New Request in top right bar

Select Request Type from the drop-down and choose Dependent

Choose the sign on the blue bar to add contacts.

Complete the information and SUBMIT the request.
