Printable Holiday Request Form

ANNUAL HOLIDAYS
The holiday year runs from 1st January to 31st December. For those employees who commence mid- year, your holiday entitlement will be calculated on a pro-rata basis.
Full-Time employees are entitled to 20 days holidays per year plus 9 public holidays. Part time employees will receive holidays on a pro-rata basis this is calculated 8% of the hours worked in the leave year, subject to a maximum of 4 weeks.

Holiday requests are not normally granted during the first three months of starting with the Company. Any holiday arrangements made before joining the Company will be honoured, but will be paid in accordance with the holiday entitlement scale. However, if these arrangements were not made known during the interview or when accepting the position offered, the Company may not be in a position to honour them.

A Holiday Request Form should be completed, submitted to and then approved by the Store Manager before you make any firm holiday arrangements. The time of taking annual holidays is subject to the agreement with the Store Manager who will try to ensure that all staff members have the dates of their choice, always considering the adequate manning of the Company, so as not to place undue burden on those working. Where two or more employees apply for the same time off and the employer deems that it is not suitable, then the granted annual leave will be on a “first come – first served basis”.

Holidays must be generally taken during the year of entitlement. At the discretion of Management, the carrying forward of part of your annual leave entitlements into the following year may be granted in unusual circumstances. However, any annual leave carried forward must be used by 31st of March of the following year. Normally no more than two weeks should be taken consecutively.

Employees are not permitted to use their annual leave entitlements from 1st July to the 31st October, the last two weeks in December and the 1st week of January. The Store Manager must approve any special arrangements for holidays. You should give at least one month’s notice of your intention to take annual leave, for shorter periods of leave of one or two days duration one week’s notice is required. If a public holiday falls on a working day during your annual holiday, you may take another day’s holiday in lieu before the end of the calendar year. Similarly, if leaving our employment, having fulfilled your contractual obligations you will be entitled to payments in lieu of untaken holidays. If, however, you have taken more than the entitlement, a deduction will be made from your final salary.

Unpaid leave of absence will only be granted in exceptional circumstances after your full holiday entitlement has been used.
