If you are under the age of 18 by the deadline, your parent or guardian MUST complete this form online. It is your responsibility to communicate this information with your parent/guardian and have them submit their form before the deadline. Please be advised that, if you are under the age of 18, failure to get this form signed by the deadline will be reported to the Provost’s Office as well as to your course instructor(s) and may result in your removal from the study/travel course.
If you are over 18 years old by the deadline, you are strongly encouraged to discuss your Study/Travel plans with your parent or guardian and have them submit the Parent/Guardian Acknowledgement of Risk Form; however, if you are over 18 years old this form is optional.
