9.2. Manual Application Form

Clubs/Service can submit nominations directly through Surfguard as an assessment request, with an application form submitted to the relevant State/Territory Centre.

How to submit an application using Surfguard

1. Submit an Assessment Request in Surguard
2. Complete the National Patrol Service Award Application Form
3. Send both the Application Form and the Surfguard Assessment Request Number to the relevant State/Territory Centre for endorsement
4. Once endorsed, the State/Territory Centre will submit the forms to SLSA
