In this tab, users can assess the root cause for the direct, detection, and systematic causes of the issue.

1. Direct Cause: This is a rich text field that allows users to input the direct cause of the issue.
2. Detection Cause: This is a rich text field that allows users to input the cause behind the detection failure.
3. Systematic Cause: This is a rich text field that allows users to input the systematic cause of the issue.
4. Submit Form: This button allows users to submit the form.
   • Once submitted, the SCAR will be sent for review.
