Ohio Revised Code 3313.671 requires that a record of immunizations for each student be on file at the school no later than day fourteen of that school year. By law, a student may be excluded from school on the 15th day of the school year if the school has not received written proof of the required immunizations (measles, mumps, rubella, diphtheria, tetanus, polio, hepatitis b, varicella/chicken pox, meningococcal disease).

The State of Ohio Legal Immunization Exemption Section 3313.671 (part 3) applies to a student whose parent or guardian presents a written statement in which the parent or guardian objects to the immunization for good cause, including religious convictions, is not required to be immunized.

The State of Ohio Legal Immunization Exemption Section 3313.671 (part 4) applies to a student whose physician certifies in writing that such immunization against a disease is medically contraindicated, and that student is not required to be immunized against that disease.

The State of Ohio Legal Immunization Exemption Form can be obtained from the CHCA Building Nurse. In the event of a communicable disease outbreak, a non-immunized student may be excluded from school, sporting events and/or any extracurricular activities.
