Did primary graft failure occur:
Indicate if primary graft failure occurred by selecting “Yes” or “No.” If Yes, continue to next question.
Primary graft failure is defined as no neutrophil recovery to ≥ 500 cells/mm3 by Day 28 post HCT. This should be confirmed by lack of donor chimerism (>5% in BM or PB), although not required to be achieved for clinical definition of primary graft failure. Lack of neutrophil recovery with donor chimerism >5% is still considered primary graft failure.
Date of graft failure:
This field will only appear in the EDC if “Did primary graft failure occur” is answered Yes. Report the date of graft failure. Date can either be entered directly in the data field using the DD/MMM/YYYY format, or the calendar date picker can be used.
